In ED specifically, Li-ESWT is used to stimulate microvascular regeneration in penile tissue. The mechanism studied in the laboratory involves mechanical stimulation of cell signalling pathways: upregulation of vascular endothelial growth factor (VEGF), recruitment of endothelial progenitor cells, and proliferation of new microvessels in the cavernosal tissue (ESSM clinical recommendations, 2019). In simpler language: it works on the biology of blood flow at the small-vessel level.

Which shockwave technology, and why the device matters
"Shockwave therapy" is used loosely in clinics worldwide, but it covers three different technologies, and they are not interchangeable for erectile dysfunction.
- Radial pressure waves (RSWT). Generated by a pneumatic projectile striking an applicator head. These are pressure waves rather than true shockwaves: the energy peak sits at the skin surface and diffuses rapidly. Radial devices are cheaper and widely marketed for ED, but the published ED trials did not use them, so results from focused-shockwave studies cannot be claimed for a radial machine.
- Electrohydraulic focused shockwaves. Generated by a spark gap inside a water-filled reflector. Effective, but with broader focal zones and more pulse-to-pulse variability, which makes reproducible dosing harder.
- Electromagnetic focused shockwaves. Generated by an electromagnetic coil and a shaped acoustic lens, focusing energy at a defined depth with reproducible energy flux density. This is the technology behind most of the higher-quality randomised trials of Li-ESWT for ED.
Hisential uses the Dornier Aries 2, an electromagnetic focused Li-ESWT platform built for sexual medicine and andrology. In practice that gives a reproducible dose at every pulse, and a focal depth that targets the corpora cavernosa and crura where new blood vessel growth needs to happen.
An honest caveat: the device is necessary but not sufficient. Protocol fidelity (pulse count, energy flux density, anatomical mapping), operator training and patient selection matter as much as the platform. What we combine is a research-grade device, a structured protocol, and an MMC-registered medical team, rather than leaning on any one of the three.
The treatment protocol and what a session involves
The protocol follows the pattern used in the published trials rather than an improvised one.
- Focused low-intensity acoustic shockwaves are delivered through a handheld applicator across defined zones on the penile shaft and crura.
- Each session runs 15-25 minutes, with no anaesthesia and no downtime. Most men resume normal activity, including exercise, straight afterwards.
- A course is 6-12 sessions, usually once or twice weekly across roughly six weeks.
- Where men notice a change, it commonly begins around the third or fourth session and continues to develop for some months after the course ends.
- The sensation is usually described as mild tapping or pressure rather than pain; topical numbing is not normally needed.

The most rigorous evidence to date is a Cochrane review of 21 randomised sham-controlled trials in 1,357 men, which found erectile-function scores improved by about 3.9 points in the short term (within 3 months) and about 5.3 points beyond 3 months, alongside better penile rigidity in the short term. The review rates the certainty of that evidence as low, because the individual trials were small and varied in method (Cochrane Database of Systematic Reviews, 2025). In plain terms: a meaningful shift for many men, not a guaranteed cure.
- If oral medication has not worked: A double-blind sham-controlled trial found that Li-ESWT moved some men who had not responded to PDE5 inhibitors back into the responder group (Kitrey et al., J Urol 2016). Some, not all - which is why response is reviewed mid-course.
- 持久性： Improvement is measured in months rather than years. Longer-term data remain limited and inconsistent, and some men see benefit fade, so a maintenance session or short repeat course is sometimes used (ESSM clinical recommendations, 2019).

The Cochrane review found little to no difference between Li-ESWT and sham treatment in treatment-related side effects or in men stopping treatment because of them (Cochrane, 2025).
Treatment is not appropriate during active infection or broken skin in the treatment field, where there is malignancy in the treatment field, or in men with bleeding disorders or on anticoagulation without case-by-case clinical judgement. Serious adverse events are uncommon when the treatment is doctor-administered with appropriate technique, energy settings, and indications.
