What ESWT actually is
Low-Intensity Extracorporeal Shockwave Therapy - Li-ESWT, often shortened to ESWT - is a non-invasive treatment that uses focused acoustic (sound) waves delivered through the skin to underlying tissue. The waves are not electric shocks; they are mechanical pressure waves at very specific energies and focal depths.
In ED specifically, Li-ESWT is used to stimulate microvascular regeneration in penile tissue. The mechanism is thought to involve mechanical stimulation of cell signalling pathways that release angiogenic factors, encouraging the formation of new microvessels and improving the function of existing ones. In simpler language: it works on the biology of blood flow at the small-vessel level.
That mechanism matters for understanding what ESWT can and cannot do. It is a tool for vascular ED - ED where the underlying problem is reduced or impaired blood flow to and within the penile tissue. It is not a hormonal treatment, a nerve treatment, or a psychological treatment. The decision about whether ESWT is the right tool for any particular man depends on identifying what is actually causing his ED.
How ESWT fits inside an ED treatment plan
ESWT is one of several evidence-based options for erectile dysfunction. The most useful way to think about it is as part of a structured plan, not as a standalone product.

- Oral medication. PDE5 inhibitors. Effective, well-tolerated for most men, taken on-demand or daily. Often the first-line option.

Combination protocols typically outperform any single approach, especially for men with longstanding ED, established vascular risk factors, or partial response to oral medication.
Who is a good candidate for ESWT
The best candidates for Li-ESWT for ED:
- Men with vascular ED, particularly mild-to-moderate severity
- Men who respond partially but not fully to oral PDE5 inhibitors
- Men who want to reduce or remove dependence on daily oral ED medication
- Men with metabolic risk factors (the same factors that drive vascular ED - diabetes, hypertension, dyslipidaemia, smoking history) where the underlying vascular biology is the target
- Men with mild Peyronie's disease (in selected cases, with careful clinical judgement)
ESWT is less appropriate or not indicated for:
- ED that is purely hormonal in origin (where treating the hormonal cause directly is the better path)
- Severe psychogenic ED with no vascular component
- Severe neurogenic ED (e.g. after radical pelvic surgery or spinal injury)
- Active penile infection or significant skin disease in the treatment area
- Significant anatomical abnormalities of the penis
- Men on anticoagulants or with bleeding disorders - case-by-case clinical judgement
- Men with pacemakers or implanted devices in the treatment field - clinical judgement required
Suitability is always confirmed at consultation after a focused history, examination, and where appropriate investigations including testosterone, fasting glucose or HbA1c, lipid panel, and PSA where age-appropriate.
What an ESWT session at Hisential is like
A typical Li-ESWT session at Hisential takes 15-25 minutes.
- Brief check-in. Confirm suitability for that session - symptoms, side effects from prior sessions if any, any new medications.
- Positioning. Comfortable, private, doctor-administered. A coupling gel is applied to the treatment area to ensure the acoustic waves transmit effectively into the tissue.
- Treatment. The focused shockwave applicator is moved across multiple defined zones, delivering a planned number of pulses at a specific energy setting. Most men describe the sensation as a mild tapping or pulsing, similar to a firm massage. Topical numbing is not usually required.
- Post-session. Normal activities can resume immediately. There is no downtime. Mild redness in the treated area is occasionally noticed and settles within hours.
A typical course is 6-12 sessions, usually once or twice per week. The exact number is set at the beginning based on severity and clinical context, and reviewed mid-course based on response.
What to expect - and not expect
Honest expectation-setting matters more with ESWT than with most ED treatments, because the mechanism is gradual and biological.
- Time to onset: Some men notice improvement within 4-6 weeks. Full effect of a course typically emerges over 2-3 months.
- Magnitude of effect: Realistic improvement, not transformation. For the right candidate, ESWT can move erectile function meaningfully - for example, from inadequate response on oral medication to good response, or from frequent need for medication to occasional use. It is not a guarantee of "cure."
- Durability: Improvement typically lasts months to years rather than weeks. Some men benefit from a maintenance session or short course at intervals.
- Best results: Achieved when the underlying drivers - vascular health, hormonal status, body composition, sleep, cardiovascular risk - are addressed in parallel. ESWT into untreated severe metabolic disease is fighting a losing battle.

Patient pathway and follow-up
A complete clinical episode at Hisential is structured around four clearly-defined stages, so patients understand what to expect at each visit and how decisions are made.
Stage 1 - Pre-consultation triage
Before your first appointment for ESWT therapy, our care coordinators take a brief history by WhatsApp or phone to determine whether teleconsultation, an in-clinic visit, or a same-day workup is most appropriate. This avoids unnecessary travel for issues that can be triaged remotely, and ensures patients with urgent presentations are prioritised. Where laboratory tests, imaging, or fasting are likely to be needed, we explain the requirements in advance so the first visit is productive rather than purely exploratory.
Stage 2 - In-clinic assessment
The first consultation includes a structured medical history, focused examination, and - where clinically indicated - point-of-care or laboratory investigations. Doctors document findings in our digital records system and walk patients through the rationale for each test. Costs are confirmed before any chargeable investigation is performed; nothing is added without consent.
Stage 3 - Diagnosis, plan, and shared decision-making
Once results are available, the doctor reviews them with the patient in plain language, including normal ranges, areas of concern, and the level of evidence behind any proposed intervention. For ESWT therapy, treatment options are framed in terms of expected benefit, realistic timelines, side-effect profile, and cost. Patients are encouraged to ask questions and may request a second appointment to consider their options before starting treatment - there is no pressure to decide on the day.
Stage 4 - Follow-up and continuity
Follow-up intervals are agreed at the end of the first visit and depend on the condition: chronic management is typically reviewed every 3-6 months, while acute episodes may be reviewed within 1-4 weeks. Teleconsultation (RM 45) is offered for routine follow-ups where physical examination is not required. Records are retained securely and remain accessible to the patient on request, including for onward referral or coordination with another doctor.
What we will not do
Hisential does not sell unproven therapies, does not recommend tests without a clinical indication, and does not extend treatment courses beyond what the evidence supports. If a condition falls outside our scope - for example, requiring inpatient care, surgical sub-specialty input, or oncology services - we say so and refer appropriately.
