STI Rash in Malaysia: Causes, Testing & Treatment

A new rash that appears on the palms or soles, spreads across the trunk, or shows as painful blisters on the genitals can be the first visible sign of an STI - most often secondary syphilis, acute HIV seroconversion, herpes or (less commonly) acute hepatitis B.   1. Does a rash on the palms and soles always mean syphilis?

    No - but it is one of the most specific clues. Hand-foot-and-mouth disease, drug eruptions and a few autoimmune conditions can also involve the palms and soles. A simple syphilis blood test rules it in or out within days, and we always recommend testing in any sexually active adult with this pattern.

  2. How soon after exposure can a rash from HIV or syphilis appear?

    The acute HIV seroconversion rash usually appears 2 - 4 weeks after exposure. Secondary syphilis rash appears later - 6 - 12 weeks after the initial painless chancre, which itself shows up 10 - 90 days post-exposure.

  3. Can I just use a steroid cream?

    Not until the cause is known. Topical steroids can dampen the rash temporarily but will not treat the underlying STI and may delay the correct diagnosis. We confirm the cause first, then use targeted treatment.

  4. Is the rash itself contagious?

    Secondary syphilis lesions and herpes vesicles are both highly infectious through direct skin contact. Acute HIV is transmitted through blood and sexual fluids, not the rash itself. Avoid sexual contact until you have been reviewed.

  5. How quickly can I be seen?
