Painful Urination (Dysuria) in Malaysia: Causes, Testing & Treatment
Painful urination (dysuria) in sexually active adults is often caused by an STI - chlamydia, gonorrhoea, NSU, Mycoplasma genitalium or herpes - rather than a simple UTI. Same-day NAAT plus urinalysis distinguishes the cause; antibiotics treat bacterial causes; antivirals control herpes outbreaks.

- Painless urination
- Mild urgency when bladder is full

- Pain referred to lower abdomen or pelvis
- Urinary frequency or urgency
- Cloudy, bloody or foul-smelling urine
- Fever or back pain (suggests upper UTI)
Common causes
Chlamydia
Common cause of dysuria in sexually active adults, often with mild or no discharge.
Gonorrhoea
Burning with yellow/green discharge in men; often silent in women.
Non-Specific Urethritis (NSU)
Urethritis without gonorrhoea - chlamydia, Mgen or unidentified causes.
Mycoplasma genitalium
Persistent dysuria after first-line antibiotics suggests Mgen.
Genital herpes (HSV)
Painful urination when urine touches genital ulcers - often with visible blisters.
Urinary tract infection (UTI)
Non-STI cause - bacterial bladder infection. More common in women but possible in men.
Prostatitis
Bacterial inflammation of the prostate - dysuria with pelvic pain in men.
When to see a doctor
Red flags
- Burning or pain that lasts more than 24 hours
- Visible blood in urine
- Fever, chills or back pain
- Discharge from the urethra or vagina
- Genital sores or blisters
- Recurrent dysuria after a previous antibiotic course
- Pregnancy with any urinary symptoms
Recommended tests
- Urine dipstick to detect UTI
- NAAT (PCR) for chlamydia, gonorrhoea, Mycoplasma genitalium on first-void urine
- Multi-site swabs if exposure history warrants
- HSV PCR swab if ulcers are present
- STI Essentials panel (HIV, syphilis, hepatitis B) if STI suspected
Treatment pathway
