What is honestly discussed: genetic testing has implications - for you, and sometimes for biological family members. Epigenetic results can be confronting if your biological age is higher than expected. We make sure you have time to think before deciding.

Is any of this covered by insurance in Malaysia?

Insurance coverage for predictive and preventive genomic testing is limited in Malaysia. We provide an itemised invoice for personal claims, but most patients pay out of pocket.

Can I share my results with my regular doctor?

Yes. Your written report is yours. We can also coordinate directly with another clinician on your behalf.

What if I find out something scary?

  1. Why screen for longevity if I feel fine?

    Most biological aging happens silently for decades before symptoms appear. Screening catches modifiable changes - accelerated biological age, dysbiotic gut, high-risk genetic variants - early enough to act. By the time symptoms appear, the prevention window is much narrower.

  2. What is the difference between whole genome sequencing and a consumer DNA test like 23andMe?

    Consumer DNA tests use a chip-based method that reads a few hundred thousand pre-selected positions in your DNA. Whole genome sequencing reads all 3 billion letters. The clinical depth is much greater, including rare disease variants that chips do not check, full pharmacogenomic profiling, and the ability to query your data for new discoveries decades from now.

  3. Why does Hisential use shotgun microbiome sequencing instead of 16S?

    16S sequencing reads only one bacterial gene and identifies microbes roughly to genus level. Shotgun metagenomic sequencing reads every DNA fragment, identifies species and often strains, captures viruses and fungi as well as bacteria, and tells us what your microbiome can functionally do - not just who's there. The clinical insight is substantially deeper.

  4. Is biological age testing reliable?

    The epigenetic clocks we use are peer-reviewed and validated. The biological age estimate is most useful as a trackable number over time. A single reading is a snapshot; the real value comes from a baseline plus a repeat 6-12 months later to see whether what you're doing is working.

  5. Will my genetic results affect my insurance?

    We do not share results with insurers under any circumstances. However, some insurance applications ask whether you have undertaken genetic testing. Speak with your concierge about timing your application relative to your testing if relevant.

  6. Can the results predict when I'll get sick or die?

    No, and we are honest about this. Genetic risk variants raise or lower probability - they are not deterministic. Biological age is an estimate, not a verdict. The value of testing is what it lets you do, not what it predicts.

  7. What if I don't want to know everything in my genome?

    Reasonable. Before testing, we discuss what categories of findings you want reported. You can choose to receive only ACMG-actionable findings (variants for which medicine has established management pathways) and decline to be told about, for example, late-onset neurodegenerative risks that have no current treatment.

  8. How often should each test be repeated?

    Genome: once in a lifetime - your DNA does not change. Epigenetic: every 6-12 months if you're actively making lifestyle changes. Microbiome: every 3-6 months if you're making dietary changes, otherwise annually.

  9. Is the gut microbiome kit easy to use at home?

    Yes. The kit contains everything needed plus written instructions. Collection takes a few minutes. The sample is shelf-stable for several days at room temperature, no refrigeration needed before drop-off or courier collection.

  10. Will I need any additional tests after the longevity screening?

    Sometimes. If a result flags something that warrants confirmation (e.g. a high cardiovascular polygenic risk score may lead to advanced lipid panel testing or imaging), your doctor will recommend the next step. These are not included in the package price and would be quoted separately.

  11. Do you offer pre-test genetic counselling?

    Yes. For the Whole Genome package, the initial consultation is structured as pre-test counselling. We walk you through implications for you and for biological family, what categories of findings you want reported, and ensure informed consent before any sample is taken.

  12. Can I do the comprehensive bundle in stages instead of all at once?

    Yes. Many patients start with one test (often Epigenetic for a current snapshot, or Genome for a lifetime baseline) and add others later. If you eventually want all three, the comprehensive bundle is cheaper than building it test-by-test.

Glossary

Biological age
An estimate of how old your body is biologically, as opposed to how many years since you were born. Measured most reliably with epigenetic clocks.
Chronological age
Years since your date of birth. A fixed number that does not reflect how well your body is functioning.
DNA methylation
Small chemical tags (methyl groups) attached to specific positions on your DNA. The pattern changes with age, lifestyle, and disease, and is what epigenetic clocks measure.
Dysbiosis
An imbalance in the gut microbial community - typically reduced diversity, overgrowth of certain species, or under-representation of beneficial ones.
Epigenome
The collection of chemical tags and structural features that sit on top of your DNA and control which genes are switched on or off in each cell.
Epigenetic clock
A statistical model that estimates biological age from patterns of DNA methylation. Examples: Horvath, GrimAge, PhenoAge, DunedinPACE.
Genome
The complete set of DNA in every cell - about 3 billion letters. Inherited from your parents and largely unchanged throughout life.
Healthspan
The portion of your life lived in good health. The goal of longevity work is to extend healthspan, not simply lifespan.
Microbiome
The collective community of microorganisms (bacteria, viruses, fungi, archaea) living in and on your body.

Polygenic risk score
