Abnormal Vaginal Discharge in Malaysia: Causes, Testing & Treatment
Changes in vaginal discharge - yellow/green colour, fishy odour, itching, or bleeding - can indicate an STI (chlamydia, gonorrhoea, Mycoplasma genitalium, trichomoniasis), bacterial vaginosis or candidiasis. Same-day NAAT and microscopy identify the cause; most are easily treated.

Normal vs abnormal
Usually normal
- Clear or white discharge varying through the menstrual cycle
- Increased clear stretchy discharge mid-cycle (ovulation)
- Mild lactic odour

Common causes
Bacterial vaginosis (BV)
Most common cause - thin grey-white discharge with fishy odour. Treated with metronidazole.
Vaginal candidiasis (thrush)
Thick white curd-like discharge with itch. Treated with antifungals.
Chlamydia
Often silent in women - may cause mild discharge, bleeding between periods, or pelvic pain.
Gonorrhoea
Increased or altered discharge with pelvic pain. ~50% of female infections are silent.
Trichomoniasis
Frothy yellow-green discharge with itch and odour. Treated with metronidazole.
Mycoplasma genitalium
Cervicitis and bleeding between periods. Resistance-guided antibiotic therapy.

Red flags
- New change in colour, consistency or odour of discharge
- Itching, soreness, redness or burning
- Pelvic or lower abdominal pain
- Bleeding between periods or after sex
- Pain during sex
- Fever or general malaise
- Symptoms during pregnancy
Recommended tests
- NAAT (PCR) for chlamydia, gonorrhoea, Mycoplasma genitalium, trichomoniasis
- High vaginal swab for BV and candidiasis (microscopy/culture)
- Pelvic examination if symptoms suggest PID
- HIV, syphilis and hepatitis B serology if STI suspected
- Pap smear / HPV testing per national cervical screening schedule
Treatment pathway
