Circumcision is the surgical removal of the foreskin (prepuce) - the layer of skin that covers the head of the penis. The procedure has been performed for thousands of years for religious, cultural, and medical reasons; modern surgical technique has made it a routine, well-tolerated procedure when performed by experienced practitioners.

In adult practice, the indication usually falls into one of three categories:

• Medical - phimosis, recurrent balanitis, lichen sclerosus, recurrent UTIs in selected cases, pre-cancerous lesions, persistent hygiene difficulty, or a painful/torn frenulum.
• Religious or cultural - adult conversion or observance.
• Personal preference - cosmetic, hygiene, or reduced-risk reasoning.

At Hisential Clinics, adult circumcision is performed using the Shang Ring (also written ShangRing) - a sterile, single-use circumcision device prequalified by the World Health Organization for adult male circumcision and used at scale in clinical programmes worldwide.

How it works: An inner ring is placed inside the foreskin against the glans, the foreskin is drawn over it, and an outer ring is clamped to compress the foreskin cleanly between the two rings. The excess foreskin is then removed. No hand-sewn sutures are required.

What happens to the rings: The outer ring is removed at the end of the procedure. The inner ring is left in place to detach naturally as the skin underneath heals - typically within 7-14 days. This is a deliberate choice. Letting the ring release on its own avoids any mechanical pulling on the healing edge and is associated with a cleaner cosmetic outcome than scheduled mechanical removal.

Why we use it: Compared with a conventional sutured circumcision, the Shang Ring approach typically means a shorter procedure, less bleeding, less post-operative pain, a more consistent cosmetic outcome, and no stitch line to manage. No hospital admission, no general anaesthesia.

Anaesthesia: Local anaesthetic only, injected at the base of the penis. You are awake throughout, feel pressure but no pain, and walk out the same day.

Duration: Typically 30-60 minutes in the procedure room, including preparation, sizing, the procedure itself, and dressing.

When this technique is not the right fit: A small number of patients with specific anatomy, advanced lichen sclerosus, certain bleeding disorders, or uncontrolled diabetes may be better served by a conventional sutured technique or onward referral. Your doctor will discuss this honestly during consultation.

A complete clinical episode at Hisential is structured around four clearly-defined stages, so you understand what to expect at each visit.

Stage 1 - Pre-consultation triage. Brief WhatsApp or phone history with your concierge to determine whether teleconsultation, an in-clinic visit, or a same-day workup is most appropriate.

Stage 2 - In-clinic assessment. Structured history, focused examination, and - where clinically indicated - investigations. Costs are confirmed before any chargeable test is performed; nothing is added without consent.

Stage 3 - Diagnosis, plan, and shared decision-making. Findings reviewed in plain language. Options framed in terms of expected benefit, realistic timelines, and side-effect profile. You may request a second appointment to consider options before scheduling.

Stage 4 - Follow-up and continuity. In-person review at 1-2 weeks post-procedure, teleconsultation thereafter as needed. Records retained securely and remain accessible to you on request.

What we will not do. Hisential does not sell unproven therapies, does not recommend tests without a clinical indication, and does not extend treatment beyond what the evidence supports. If a case falls outside our scope, we say so and refer appropriately.

Detailed written aftercare instructions are provided on the day of your procedure. The general principles:

• Wound care. Keep clean and dry. Gentle showering from day 1; avoid baths, pools, and sea until fully healed. Pat dry rather than rub.
• Dressings. A simple non-adherent dressing is usually applied. You'll be told when and how to remove or change it.
• Activity. Light walking from day 1 is encouraged; avoid heavy lifting, cycling, intense exercise, and prolonged standing for the first 1-2 weeks. Office work usually fine from day 3-4.
• Medication. Paracetamol regularly for the first 48-72 hours, with an NSAID (if not contraindicated) for additional comfort. A short course of oral antibiotic is occasionally prescribed depending on the case.
• Erections. Nocturnal erections in the first week can tug uncomfortably as the skin tightens; this settles as healing progresses.
• Sexual activity is deferred until 4-6 weeks post-procedure to allow complete healing.
• Shang Ring. The inner ring is left in place and detaches naturally as the skin heals, typically within 7-14 days; no removal visit required.
• Follow-up. In-person review at 1-2 weeks; teleconsultation thereafter as needed. WhatsApp the clinic with any questions between visits.

When to contact the clinic promptly: heavy or persistent bleeding, fever, increasing pain after day 3, spreading redness, pus-like discharge, or inability to urinate. These are uncommon but should be assessed quickly.
