Why men are the underserved half of HPV vaccination
HPV is not only a women's health issue. Most sexually active adults will acquire HPV at some point, and in men HPV causes genital warts and a rising share of penile, anal and throat cancers. Oral HPV infection in men has increased markedly over the past two decades (Chaturvedi et al., Journal of Clinical Oncology, 2011), and the majority of HPV-related throat cancer cases occur in men. Yet Malaysia's national school programme, running since 2010, vaccinates 13-year-old girls only. Adult men are left to seek vaccination privately, and most hospital programmes market to women aged 15 to 45. Hisential vaccinates men and adults on the same evidence base used everywhere else: a randomised trial in over 4,000 men showed the quadrivalent HPV vaccine prevented about 90 percent of HPV-related external genital lesions in men aged 16 to 26 (Giuliano et al., New England Journal of Medicine, 2011).
What Gardasil 9 covers
Gardasil 9 protects against nine HPV types: 6, 11, 16, 18, 31, 33, 45, 52 and 58.

A large randomised trial confirmed Gardasil 9's efficacy against disease from the five additional types (Joura et al., New England Journal of Medicine, 2015), and WHO recommends HPV vaccination as part of national immunisation programmes (WHO position paper, 2017 and 2022 update).
Who can be vaccinated (age guidance)
- Under 15: two doses, at least 6 months apart - the standard schedule, strongest immune response.
- 15 to 26: three doses, at 0, 2 and 6 months - the catch-up window with the clearest benefit.
- 27 to 45: three doses, same schedule - recommended case by case. The US CDC describes this as a shared clinical decision between you and your doctor, because prior exposure reduces - but does not eliminate - the benefit. If you are in a new relationship, previously unvaccinated, or have had warts treated, vaccination still protects against HPV types you have not yet encountered.
- Men who have sex with men: vaccination is recommended up to 45, as HPV-related anal cancer risk is higher in this group.
What the vaccine can and cannot do
The vaccine prevents new HPV infections. It does not treat an existing infection, and it does not remove warts you already have. Understanding HPV (brief context for adults)
HPV (human papillomavirus) is very common. Most sexually active adults encounter it at some point, and in the large majority of cases the immune system clears the infection on its own within about 2 years without ever causing symptoms (WHO fact sheet, 2024). It is persistent infection, not first exposure, that carries longer-term risk.
Transmission is skin to skin, including sexual contact, and it can happen without visible warts or any symptoms at all. Condoms reduce transmission but do not fully prevent it, because HPV can be present on skin the condom does not cover.
This is why vaccination plus screening beats either alone. Vaccination closes the door on the HPV types you have not yet encountered, while a full health screening picks up what is already present and tracks the rest of your preventive picture over time.
