STI Rash in Malaysia: Causes, Testing & Treatment
A new rash that appears on the palms or soles, spreads across the trunk, or shows as painful blisters on the genitals can be the first visible sign of an STI - most often secondary syphilis, acute HIV seroconversion, herpes or (less commonly) acute hepatitis B. A targeted blood and swab panel identifies the cause; most are highly treatable when caught early.

- Transient flushing or heat rash that fades within hours
- Mild contact reaction to a new soap, detergent or fabric
- A few isolated acne spots that come and go

- Diffuse pink-red rash across the trunk with flu-like illness
- Painful clustered blisters on the genitals, buttocks or mouth
- Itchy hives appearing alongside fatigue, joint pain or jaundice
- Any rash that follows a recent unprotected sexual exposure

Secondary syphilis - copper-coloured spots on the palms and soles. Secondary syphilis
A painless, often symmetric rash on the palms, soles or trunk appearing 6 - 8 weeks after the initial chancre. Curable with an in-clinic antibiotic injection.

Acute HIV - diffuse trunk rash with flu-like illness 2 - 4 weeks after exposure. Acute HIV seroconversion
A diffuse pink-red maculopapular rash across the upper trunk 2 - 4 weeks after exposure, usually with fever, sore throat and lymph node swelling. 4th-generation HIV test confirms.

Herpes - grouped vesicles on a red base, typically painful or tingling. Herpes (HSV)
Painful grouped fluid-filled vesicles on a pink base, often preceded by a tingling or burning prodrome. Diagnosed by PCR swab of a fresh lesion.

Acute hepatitis B - urticarial rash can accompany fatigue and jaundice. Acute hepatitis B
Less common - itchy urticarial wheals or a serum-sickness rash during the acute phase, often with fatigue, nausea and jaundice. Hepatitis B serology confirms.
Non-STI mimics
Scabies, contact dermatitis, drug eruption, tinea cruris (jock itch) and viral exanthems can all mimic an STI rash. A clinical exam plus a focused STI panel distinguishes them.

- Rash involving the palms and soles
- Painful blisters or ulcers on the genitals, buttocks or mouth
- Rash with fever, sore throat, swollen lymph nodes or jaundice
- Rash that spreads rapidly or fails to settle within a week

- 4th-generation HIV antigen/antibody test from 18 days post-exposure

- Full STI Essentials panel including chlamydia/gonorrhoea NAAT

No - but it is one of the most specific clues. Hand-foot-and-mouth disease, drug eruptions and a few autoimmune conditions can also involve the palms and soles. A simple syphilis blood test rules it in or out within days, and we always recommend testing in any sexually active adult with this pattern.
