Does a rash on the palms and soles always mean syphilis?

No - but it is one of the most specific clues. Hand-foot-and-mouth disease, drug eruptions and a few autoimmune conditions can also involve the palms and soles. A simple syphilis blood test rules it in or out within days, and we always recommend testing in any sexually active adult with this pattern.

A new rash that appears on the palms or soles, spreads across the trunk, or shows as painful blisters on the genitals can be the first visible sign of an STI - most often secondary syphilis, acute HIV seroconversion, herpes or (less commonly) acute hepatitis B. Acute hepatitis B - urticarial rash can accompany fatigue and jaundice.
Acute hepatitis B
Less common - itchy urticarial wheals or a serum-sickness rash during the acute phase, often with fatigue, nausea and jaundice. Hepatitis B serology confirms.
Non-STI mimics
Scabies, contact dermatitis, drug eruption, tinea cruris (jock itch) and viral exanthems can all mimic an STI rash. A clinical exam plus a focused STI panel distinguishes them.
